The online forms MUST be filled out by all students (grades 7-12) who wish to participate in any sport/athletic class on their respective campus.

There are three ways to start the online forms:

• Log on with your Rank One account
• Create a new Rank One account
• Continue as guest

Click on each of the 4 forms individually to fill out.

Sign each form as requested in the signature box provided.

You must click on all links to forms and manuals if provided, in order to continue.

Add an email at the bottom of each form and you will receive a confirmation on each completed form.

Students are required to use the Pre-participation – Physical Examination Form provided. NO OTHER PHYSICAL EXAM FORM WILL BE ACCEPTED. A new physical is required for all student-athletes, prior to the start of the new school year.

If your son/daughter, deemed by a physician, requires any medication to participate in sports, please present that information to the athletic trainer of that campus.
